We know that facing cancer is hard enough without the added stress of medical bills. The 3929 Foundation is here to help, and we want to make the application process as smooth as possible for you. Please read the following carefully before starting the form below.
Fill out the form completely. Every section of the application must be completed to be considered for assistance. We are unable to process applications that are missing information or required documents.
Upload your actual bills. To pay your bills, we need the actual bill documents. Please upload the original PDF or a clear image file of each bill. We do not accept screenshots of bills, and we do not accept photos of a computer or phone screen showing a bill. Applications with bills that are not uploaded properly cannot be processed.
Provide proof of your diagnosis. In the Proof of Diagnosis field, upload a document from your medical provider that confirms your cancer diagnosis.
Provide treatment verification. In the Treatment Verification field, upload a document from your medical provider that confirms your current treatment.
Show your need for assistance. Please include documentation that shows your need for financial aid.
What happens next Once you submit your application, our review process can take up to 4 to 6 weeks. We will reach out to let you know whether your application has been accepted.
